About Trillium Health Partners
Trillium Health Partners is creating a new kind of health care – defined not by illness, but by the health and well-being of people and communities.
As one of Canada’s largest community-based teaching hospitals, Trillium Health Partners serves the growing and diverse populations of Mississauga, West Toronto, and surrounding communities through three main sites, the Credit Valley Hospital, the Mississauga Hospital, and the Queensway Health Centre, and a network of community and digital partnerships.
Through Trillium HealthWorks, the largest health infrastructure renewal in Canada’s history, we are building The Peter Gilgan Mississauga Hospital and the Shah Family Hospital for Women and Children - together, the largest hospital in Canada - and expanding the Queensway Health Centre, which will become The Gilgan Family Queensway Health Centre.
Our care extends beyond hospital walls. We work hand in hand with patients, families, and community partners to ensure care is seamless, equitable, and designed around the people we serve.

Credit Valley Hospital
Core services
The Credit Valley Hospital delivers emergency care, surgery, inpatient medicine, critical care, mental health, diagnostic imaging and lab services for patients across the community.
Specialty services
The Credit Valley Hospital is recognized for its specialized care in women’s and children’s health, renal services, clinical genetics, and regional cancer care through the Carlo Fidani Regional Cancer Centre.
Mississauga Hospital
Core services
The Mississauga Hospital delivers emergency care, surgery, inpatient medicine, critical care, mental health, diagnostic imaging and lab services for patients across the community. It is the future site of The Peter Gilgan Mississauga Hospital and the Shah Family Hospital for Women and Children, together forming the largest hospital in Canada and setting a new standard for digitally connected patient-centred care.
Specialty services
The Mississauga Hospital is home to highly specialized regional programs including cardiac health, stroke, vascular and neurosurgery.
Queensway Health Centre
Core services
The Queensway Health Centre provides outpatient and ambulatory care, Urgent Care Centre, day surgery, complex continuing care and rehabilitation. It is also the future site of The Gilgan Family Queensway Health Centre.
Specialty services
The centre is home to the Urgent Care Centre and one of the largest freestanding day surgery centres in North America, with specialized programs in orthopedics, rehabilitation, cardiac wellness and recovery, seniors’ care, and cancer screening and outpatient treatment.

Peel Behavioural Services
Peel Behavioural Services is a community service that offers functional behavioural assessments, behaviour support plans, brief interventions, consultative assistance, social skills groups, and parent training.
Mental Health and Addiction Services
Trillium Health Partners’ mental health program provides comprehensive, patient-centred and accessible care for people at different stages of their lives who are experiencing mental health or addictions issues. Services include acute care and inpatient services, ambulatory and outpatient services, child and adolescent services, medical psychiatry, and seniors' mental health services.
Multi-Care Kidney Clinic
The Multi-Care Kidney Care Clinic supports people living with advanced chronic kidney disease (stages 4 and 5) to stay as healthy as possible and delay disease progression. A dedicated, multidisciplinary team helps patients and families understand treatment options, prepare for dialysis, and plan for future care.
Reactivation Care Centres

The Church Street Reactivation Care Centre supports patients who no longer require acute hospital care but are waiting for their next care setting, such as convalescent or long-term care. Patients receive therapy, rehabilitation, and activation supports in a quiet, community-based setting that helps them build strength, regain independence, and prepare for their next stage of care.

The THP–UHN Reactivation Care Centre brings together Trillium Health Partners’ inpatient rehabilitation services in a purpose-built facility at 82 Buttonwood Avenue in West Toronto. Supported by highly skilled interprofessional teams and on-site specialized services, the centre provides high-quality, coordinated care in a calm, healing environment designed to support recovery and independence.
Insight Health Solutions
Insight Health Solutions, a part of Trillium Health Partners, supports organizations in reducing the risk and cost associated with the impacts of injury, disability and work-related absences. Evidence based, best in class programs result in superior results and client satisfaction regionally and across Canada.
Diabetes Management Centre
The Diabetes Management Centre supports the community through education, counselling, and specialized care. A multidisciplinary team helps people with prediabetes, gestational diabetes, and Type 1 and Type 2 diabetes manage their health and live well.
South Mississauga Dialysis Centre
Located adjacent to Wellbrook Place, the South Mississauga Dialysis Centre operates 16 stations for long-term care residents and community members living with chronic kidney disease. Staffed by Trillium Health Partners’ nephrologists, nurses, and multidisciplinary team members, the centre helps provide a collaborative care model for residents and community members requiring dialysis.
