About THP-UHN RCC
To support the highest quality of patient care, Trillium Health Partners (THP) is collaborating with the Ministry of Health, and other health system partners to expand hospital capacity in the GTA. With this, THP is integrating all its inpatient rehabilitation services to a designated space at the University Health Network Reactivation Care Centre (THP-UHN RCC), located at 82 Buttonwood Avenue in West Toronto. This move will bring together THP’s rehab services in one location to provide high quality, coordinated care in an environment designed to enable recovery. The program will relocate to its permanent home and centre of specialized care at The Gilgan Family Queensway Health Centre, scheduled to open in 2029.
THP-UHN RCC is part of THP’s interconnected system of care. It offers a healing environment supported by exemplary care teams and a variety of specialized services designed to help patients gain their highest level of independence.
Excellence in Rehabilitation and Recovery Services
THP’s highly skilled, interprofessional rehabilitation team works together in a supportive setting with access to on-site specialized services to deliver high quality, coordinated care for patients impacted by disease or injury. This purpose-built facility is located within a residential neighbourhood near Jane Street and Eglinton Avenue, where quiet surroundings and green space extend a calm and healing environment.
