Short Term Case Management Pathway (STCM)

This program will provide short term, time limited and task oriented case management to individuals being discharged from the Inpatient Psychiatry Unit who are diagnosed with a psychotic or mood disorder. Referrals will also be accepted from the Crisis Intervention Team of the Trillium Health Centre. The intent of these referrals is to meet the short-term needs of individuals experiencing increased levels of recidivism as illustrated through their contact with the THP Emergency Department, as well as our Inpatient Programs. The purpose of this program is to assist the client's reintegration into the community through acquisition and linkages of needed and appropriate community supports and services.

The short-term case manager will work with the client, the referral source, significant others as is appropriate and the targeted services to facilitate the recovery process. This may include, but is not limited to facilitating financial assistance, connecting to a psychiatrist and general practitioner, engaging in the transition process to appropriate hospital and community based outpatient services, and making referrals as necessary to resources such as housing and social recreational programs.

Criteria

  • Resides in Mississauga/Halton LHIN
  • Over the age of 16
  • Primary diagnosis of Psychotic or Mood Disorder
  • Poses no imminent danger to self or others
  • Identifiable needs which can be met through task oriented, short term interventions. These may include the areas of housing, finances, medical, and social
  • Client consents to short term case management services with a minimum of one visit per week in the community

Exclusionary Criteria

  • Community visits pose risk to staff safety
  • Acutely suicidal
